Enclosed is a non-refundable $100 or $200 reservation deposit. Balance will be paid upon arrival at camp. All deposits must be received no later than May 28, 2010. Anyone sending the deposit after this date may be charged a $50.00 late fee; with special approval this may be waived. Camp cost includes all instruction, room and board, and camp T-shirt.
After receipt of your application, a confirmation letter including specifics on the camp will be mailed to you approximately 1 month prior to camp.
PROOF OF INSURANCE FORM AND PERMISSION FORM
It is MANDATORY that all campers/coaches carry their own medical insurance and this form is signed. Please fill out the appropriate information and check that you agree to this information. NOTE: No one will be allowed to attend camp without this Proof of Medical Insurance.
ASSUMPTION OF RISK AGREEMENT TO HOLD HARMLESS AND EMERGENCY RELEASE FORM
As the parent/legal guardian of the camper (coach*) named here
participant, it is understood that playing or participating in any sport can be a dangerous activity involving MANY RISKS OF INJURY. It is understood that the dangers and risks of playing or practicing to play/
participate in wrestling include, but are not limited to, death, serious neck injury or impairment to other aspects of the body, general health and well being. It is
understood that the dangers and risks involved with playing or participating in wrestling may result not only in serious injury, but in a serious impairment of one’s
future abilities to earn a living, to engage in other business, social and recreational activities, and generally to enjoy life.
The following additional activities are engaged in at the Rocky Mountain Wrestling Camps (RMWC). Each participant may be involved in any one of these activities.
Each of these activities also has certain inherent risks similar to those mentioned above. These activities include but are not limited to: running, hiking, fishing,
transported by vehicle, swimming, basketball, weight lifting, racquetball, softball, frisbee, touch type football, volleyball, and more. White water rafting trips are
available to those groups wishing to pursue them from local guides. This is NOT organized or sanctioned by the RMWC.
By signing below, the participant and parent/legal guardian hereby assume all risks associated with participation and agree to hold Western State College, the Rocky
Mountain Wrestling Camp (RMWC), RMWC Director and all agents, coaches, and volunteers harmless from any and all liability, actions, causes of action, debts, claims,
or demands of any kind and nature whatsoever which may arise by or in connection with participation in any activities related to the RMWC. The terms hereof shall
serve as a release and assumption of risk for your heirs, estate, executor, administrator, assignees, and for all members of the family.
Additionally, by signing below the participant and parents/legal guardian understand, in the event of an emergency every effort will be made to contact them.
However, in the event they cannot be reached, this form hereby gives permission to the physician selected by the RMWC to hospitalize and secure proper treatment
(including surgery) for the participant.
Moreover, by the parent AND participant signing this form, they are agreeing to abide by all camp rules and reasonable authority of the camp staff and that the
participant has been deemed physically able to participate in wrestling and recreational activities by a physician.
* Coaches are signing as their own self-guardian.
